Computer-Related Incidents with Commercial Aircraft

Philippine Airlines A320, Bacolod, Philippines

Synopsis The aircraft landed at Bacolod with the thrust lever for the #1 engine left in the Climb detente. Reverse thrust was applied on the other engine. Spoilers did not deploy (it requires both thrust levers to be in idle, or reverse, or one in reverse and one in idle, to do so). The aircraft departed the runway to the side, and was rbrought back to the runway with an application of forward thrust on #2, on which reverse thrust had already been commanded. The aircraft overran the runway and hit residential buildings. Three people on the ground, but no one on the aircraft, died. Here is the Report.
